Waiver Requirements

By participating in Ubuntu Legacy Community Care programs, you acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in programs may involve physical activity and certain inherent risks. I voluntarily assume all risks associated with participation.

2. Release of Liability

I release Ubuntu Legacy Community Care, its directors, officers, employees, and volunteers from any and all liability for injuries or damages that may occur during program participation.

3. Medical Authorization

In case of medical emergency, I authorize Ubuntu Legacy Community Care staff to seek medical treatment on my behalf and agree to be responsible for any associated costs.

4. Photo and Media Release

I grant permission for Ubuntu Legacy Community Care to use photographs and videos taken during programs for promotional and educational purposes.

5. Code of Conduct

I agree to conduct myself in a respectful manner and follow all program guidelines and instructions from staff and volunteers.
